First Dedicated FERTILITY & MALE Health Center with Highest Qualified Staff and Success Rates in Pakistan
Are you struggling with male infertility or failed fertilisation? At SAWA Perfect Fertility Clinic in Lahore, we offer advanced ICSI treatment in Pakistan – a proven, scientifically precise solution that gives couples the best possible chance of achieving pregnancy. Our internationally trained specialists combine cutting-edge microinjection technology with compassionate, personalised care.
ICSI (Intracytoplasmic Sperm Injection) – is a highly advanced form of IVF (In Vitro Fertilization) in which a single, carefully selected healthy sperm is injected directly into a mature egg using a microscopic glass needle. Unlike conventional IVF, where sperm must penetrate the egg on its own in a laboratory dish, ICSI eliminates this barrier entirely.
This revolutionary technique was first successfully performed in 1992 and has since transformed the treatment of severe male infertility worldwide. Today, ICSI accounts for over 70% of all IVF cycles globally. At SAWA Perfect, it is performed by our internationally trained embryologists using Pakistan’s most advanced micromanipulation equipment.
Key Difference: In standard IVF, thousands of sperm compete to fertilise one egg. In ICSI, our embryologist personally selects the single best-quality sperm and injects it directly – dramatically improving fertilisation chances even when sperm count is extremely low or sperm quality is poor.
At SAWA Perfect Fertility Clinic in Lahore, ICSI treatment is performed within our state-of-the-art Advanced Semenology Lab – the first of its kind in Pakistan – ensuring the most accurate sperm assessment and the highest-quality embryos for transfer.
ICSI is the recommended treatment for couples experiencing:
Low Sperm Count (Oligospermia) : Fewer sperm than required for natural conception or standard IVF. Even a very small number of sperm is sufficient for ICSI.
Poor Sperm Motility (Asthenospermia) : Sperm that cannot swim well enough to reach and fertilise an egg naturally or in a conventional IVF dish.
Abnormal Sperm Shape (Teratospermia) : A high percentage of abnormally shaped sperm that reduces natural and standard IVF fertilisation chances.
Obstructive Azoospermia (No Sperm in Ejaculate) : No sperm present in ejaculate due to blockage or production failure. Sperm is surgically retrieved via TESA, PESA, or MicroTESE and used directly in ICSI.
Previous IVF Fertilisation Failure : Couples who experienced failed or poor fertilisation in prior conventional IVF cycles. ICSI can overcome this barrier.
Unexplained Infertility : When no clear cause is identified, ICSI maximises fertilisation potential and removes sperm penetration as a variable.
Surgically Retrieved or Frozen Sperm : ICSI is compatible with sperm retrieved surgically or previously frozen – making it essential for azoospermia cases.
Pre-implantation Genetic Testing (PGT) : ICSI is required before genetic testing of embryos to prevent contamination from extra sperm in the dish.
Your journey begins with a comprehensive assessment including detailed medical history, hormone blood tests, pelvic ultrasound for the female partner, and a full Advanced Semenology analysis for the male. Our specialists identify the precise cause of infertility and confirm whether ICSI is the right treatment for you.
The female partner receives carefully dosed hormonal injections – typically for 10 to 14 days from day 2 or 3 of her cycle – to stimulate the ovaries to develop multiple mature eggs. Regular monitoring through blood tests and ultrasounds tracks follicle growth and guides medication adjustments throughout.
Once follicles reach optimal maturity (typically 18–22mm), a trigger injection is administered to prepare eggs for final maturation. Egg retrieval is scheduled precisely 34–36 hours after this injection for maximum success.
Under mild sedation, our fertility specialist uses a fine ultrasound-guided needle to gently aspirate mature eggs from the ovarian follicles. The procedure takes approximately 20–30 minutes. Retrieved eggs are immediately transferred to our Advanced Embryology Lab for preparation.
On the same day as egg retrieval, a sperm sample is collected. Our Advanced Semenology Lab – Pakistan’s first – conducts state-of-the-art analysis to identify and select the single best-quality sperm for injection. For azoospermia cases, sperm is surgically retrieved via TESA, PESA, or MicroTESE.
This is the defining step. Our expert embryologist immobilises a single healthy sperm, then injects it directly into the cytoplasm of each mature egg using a micro-fine injection needle under a powerful micromanipulation microscope. The injected eggs are immediately returned to incubation.
Fertilised eggs (now embryos) are cultured in our advanced incubators for 3 to 5 days. Our embryologists monitor development daily, grading embryo quality. Day 5 blastocyst culture is available for improved embryo selection. Surplus healthy embryos can be frozen (vitrified) for future cycles.
The highest-graded embryo(s) are gently transferred into the uterus using a fine catheter guided by ultrasound. This is a painless, sedation-free procedure lasting just a few minutes. The number of embryos transferred is determined by age, embryo quality, and medical history.
Approximately 14 days after embryo transfer, a blood beta-hCG test confirms whether implantation has been successful. A positive result is followed by a confirmatory ultrasound scan 2 weeks later to check for a foetal heartbeat.
Both ICSI and conventional IVF are assisted reproductive technologies where fertilization happens outside the body in a laboratory, but the key difference lies in how the sperm fertilizes the egg.
Eggs and sperm are simply placed together in a dish and left to fertilize naturally on their own. The sperm must swim to and penetrate the egg without assistance. This works well when sperm quality and quantity are normal.
A single healthy sperm is selected by an embryologist and injected directly into the egg using a microscopic needle. This bypasses all natural barriers to fertilization entirely.
ICSI fertilisation rates in Pakistan range from 70 to 80% per injected mature egg. Clinical pregnancy rates per cycle range from 40 to 65%, depending significantly on the female partner’s age, egg quality, sperm quality, and uterine health. At SAWA Perfect, our personalised protocols are designed to maximise your specific chances of success.
Fertilisation Rate: 70–80% of mature eggs successfully fertilised per ICSI cycle
Clinical Pregnancy Rate (Under 35): 50–65% per cycle for women under 35 years of age
Live Birth Rate (35–40): 35–50% per cycle for women aged 35 to 40 years
Day 5 Blastocyst Culture: Available at SAWA Perfect for higher-quality embryo selection

For male factor infertility or previous fertilization failures, ICSI involves injecting a single healthy sperm directly into each mature egg, significantly improving fertilization rates.
We offer vitrification technology for freezing surplus embryos, allowing future pregnancy attempts without repeating the entire IVF cycle. Frozen embryo transfer cycles have success rates comparable to fresh transfers.
Extended embryo culture to day 5 (blastocyst stage) allows our embryologists to select the highest quality embryos for transfer, improving implantation rates and reducing multiple pregnancy risks.
